Notice is hereby given that bids will be received at the office of the Madison County Commissioners, 1 North Main Street, London, Ohio, 43140, on Tuesday, August 14, 2018 at 10:30 A.M., Ohio Standard Time for the purpose of APPLICATION OF PAVEMENT MARKINGS ON VARIOUS ROADS WITH TRAFFIC PAINT (2018 Madison County Pavement Marking Program) according to specifications on file in the Madison County Commissioners’ Office.

 

Bids shall be submitted on forms furnished by the Madison County Engineer in a sealed envelope marked “2018 Madison County Pavement Marking Program” on the outside.

 

Bid packets may be obtained from the Madison County Engineer, 825 US 42 NE, London, Ohio, 43140 or can be downloaded from http://sharepoint/publicaccess/default.aspx or under “Notices” on the Madison County website at www.co.madison.oh.us.  Contact the Madison County Engineer’s Office at 740-852-9404 for more information.

 

Bidders must be ODOT Pre-Qualified:  “Work Type 45-Pavement Markings” and shall be pre-qualified at the time of the bidding, at the time of award, and throughout the life of the construction contract.

 

Bids will be furnishing labor, equipment, and materials.

 

Bids shall be unit price and total price.

 

As specified in R.C. 153.54, a bidder must submit a bid guaranty in the form of either:

(1) a bond for the full amount of the bid,  or

(2) a certified check, cashier’s check, or letter of credit in the amount of 10% of the bid.

 

Bidders shall comply with the provisions of the Americans with Disabilities Act of 1990.

 

Bids will be awarded to the lowest and best bidder, based on the grand total of the Unit Price Bid.

 

The Board of Commissioners reserves the right to reject any or all bids and to waive any defects in the bids.

Request for Proposals

Madison County Department of Job and Family Services (MCDJFS) intends to enter into transportation contracts to provide transportation for our clients to medical and other miscellaneous appointments (outpatient, testing, etc.). Contracts will be open to any providers with appropriate qualifications, regular vehicles, wheel chair accessible vehicles, ambulatory services, etc. Bidders interested in competing for the awards of these contracts must submit two (2) copies of a sealed proposal labeled "MCDJFS Transportation RFP" to: Madison County Commissioners, at their office at 1 N. Main St., PO Box 618, London, Ohio 43140, on or before Friday, May 25, 2018 at 4:00pm. Proposals will be opened and notated at the Madison County Commissioner’s office on Tuesday, May 29th, 2018 at 10:30 a.m., and reviewed thereafter at the Madison County DJFS office. The proposal must outline in detail all aspects of the bidder’s system and their ability to meet the requirements of this contract and contain sufficient information for MCDJFS to make an informed decision of the quality of costs of the services proposed. Bridges Community Action Partnership seeks bids to construct an office area of approximately 5,000 square feet within existing building located 284 Lafayette St, London, OH 43140 per architect’s approved design plans and new flooring schedule. HVAC to be done by others, do not include in bid. Bid to be itemized as follows:

(1)   WALLS: complete wall assembly from framing to finished paint including DW wrapped supports, installed and finished doors with hardware.

(2)   BREAK ROOM: wall and base cabinets with sink including plumbing supply and drain lines. See sheet A1.1 drawing B as reference.

(3)   ADA compliant restrooms retrofit including accessibility signage and mop sink complete

(4)   REFLECTIVE CEILING PLAN: all ceiling work as shown on sheet A8.1 to include new lighting fixtures and relamping of existing on sheet E1.1

(5)   ELECTRICAL: all electrical as designed.

(6)   REAR EXIT: threshold/stoop and rear door work see sheet A1.0 drawing C and keyed note 3.

(7)   FLOORING: per addendum schedule.

Bid should also include all permits and building fees. Bid package must also include Certificate of Ohio Workers’ Compensation insurance and Certificate of Liability insurance.

Guaranteed completion time will be weighed strongly as a factor in acceptance of successful bid

The tax shall be levied upon the grantor named in the deed and shall be paid by the grantor for the use of the county to the county auditor at the time of the delivery of the deed as provided in section 319.202 of the Revised Code and prior to the presentation of the deed to the recorder of the county for recording.
